Position Description

Position Title: Family Advocate

Program: Family Connect

Case Manager III

Reports to: Program Director

Position Function/Overview:

This position is responsible for providing counseling, information, referral, advocacy to youth and their families as well as active facilitation and participation in family life. The position involves home visits and work with clients in residential treatment settings.

Primary Duties/Responsibilities:

  • Provide facilitative counseling, information, referral and advocacy to youth and their families.
  • Provide crisis intervention counseling to families in crisis.
  • Provide on-call crisis intervention services to clients.
  • Provide a comprehensive assessment of families' strengths and needs, and psychosocial history.
  • Work with client families in developing their Family Preservation Plan through constructive, active participation in family life.
  • Teach family life skills, including budgeting, nutrition, personal hygiene, household maintenance and purchasing.
  • Teach and assist parents to meet the physical, mental and emotional needs of their children.v
  • Assist clients in linkage to referrals within the community.
  • Assist clients in identifying and accessing necessary supports in the form of in-kind and other community resources.
  • Work with city recreation staff to plan and implement "Family Fun Nights" at city facilities.
  • Provide follow-up on closed cases.
  • Attend community meetings as necessary in order to facilitate client service delivery.
  • Compile required reports on families on caseload in a timely and accurate manner.
  • Other duties as assigned or directed by Supervisor.

Qualifications Required:

  • BA degree in human services field.
  • Two years experience working with families with children.
  • Demonstrated interpersonal and communication skills.
  • Willingness and ability to travel locally, work a flexible schedule which includes evenings and possibly weekends, and work in a home-based environment.
  • Ability to work effectively with individuals from a variety of cultural and ethnic backgrounds.
